Prescribed and non-prescribed Over the Counter Medications shall be administered only after a medication order is completed and signed by the physician and parent.

If a student requires a special diet accommodation at school, the special diet order form must be completed and signed by a physician and the parent.

All dietary and medication orders are valid for one school year.
